Thank you for choosing UAB Eye Care for your vision health needs. We recognize that there are many qualified Optometrists in Alabama, and we appreciate you entrusting UAB Eye Care with your vision health. We understand how difficult it can be to deal with billing issues at the same time you’re dealing with healthcare needs. So, we make every attempt to make the process as easy as possible. The information in this section is designed to help you understand your bill and answer a few common billing questions..


Payment Policy

 

Healthcare costs are often unanticipated and not all costs are covered by your vision plan or medical insurance. Please know that we have payment plans and, in certain circumstances, financial assistance programs that can make your payments more manageable. As with any healthcare provider, it is our policy to collect all monies owed to UAB Eyecare from either your insurer or from you. Please understand that not all services provided by UAB Eye Care will be covered by your insurer and you are responsible for those costs. For non-emergent visits, your treatment may be denied or delayed if you have an outstanding balance. As noted above, we may be able to assist you in managing your health care expenses. Talk to us before you arrive so that your appointment date is easy and hassle-free.

Patients with Insurance Coverage

 

Please be advised that there may be certain services provided by UAB Eye Care which are not covered by our contract with your health plan. Please inquire with your vision plan before calling to schedule an appointment. When you visit us, please bring the proper insurance identification cards so we can bill your insurance carrier. If you are required to file personally for your company’s insurance, we will provide you with all necessary forms to file your own claim. When you visit, you will be expected to pay any charges that will not be reimbursed by insurance (e.g., co-payments, non-covered services), unless you have made prior arrangements with one of our Patient Account representatives. Because your insurance coverage is a contract between you and your insurance company, it is your responsibility to obtain any physician referral and/or precertification that your insurance company requires in order for your visit to be covered. You should also be aware that your visit may include procedures that are not covered by your Vision Plan but are covered by your Medical Plan. Under such circumstances, you will have to pay a co-pay for both your Vision Plan and Medical Plan. Oftentimes, medical procedures are not known at the time of check-in so you will be asked to make a co-pay against your medical plan during check-out.

Non-Covered Services

 

Any services that are not covered by your existing insurance policy will require payment in full at the time of service or upon receipt of denial from your insurance carrier. We may also be able to work with you on a case-by case basis to set up a payment plan for non-covered procedures.

Patients without Insurance Coverage / Financial Assistance

 

If you do not have health care coverage, you will be asked to pay the full amount of your estimated charges at the time of service. If you are unable to pay for necessary vision care, you may qualify for financial assistance. To determine if you are eligible for financial assistance, we will ask you to speak to a Financial Advocate. You may contact a Patient Account representative directly at 205-934-3899.
